Set up your office account

To set up your account as an office or clinic for the first time:

  1. Find the invitation email or SMS text message sent to you during onboarding
    • The invitation includes your username and a link to set your password.
  2. Select the Set Password link
    • If the link expired or you only have a username, go to screenpatients.com Link to an external site., select Forgot password? and follow the prompts to send yourself a new set password link.
  3. Set a password for your account
  4. Login to screenpatients.com Link to an external site. with your new password and your username from the invitation

Now you can prepare and send screening referrals.
