Before screening appointment#
Log into your patient portal and fill out draft forms shared by the Office.
☐ Portal Account: For your first screening, create your patient portal account from the invite the Office sent.
☐ Log In: Log into your patient portal at screenpatients.com 
☐ Screening Form: Fill out the draft copy in your patient portal.
☐ Patient Information Form: Fill out the draft copy in your patient portal.
Note: Repeat Screenings#
If you filled out a Patient Information Form before, the Office might not ask you to fill out a new copy.
